If you have questions, or need assistance, please contact Marina Gano, Graduate Program Manager (<a href=\"mailto:uwsopchoice@uw.edu\">uwsopchoice@uw.edu)<\/a> or Dr. Douglas Barthold, Associate Professor &amp; Chair of the Admissions Committee (<a href=\"mailto:barthold@uw.edu\">barthold@uw.edu<\/a>).<\/p>\n<h4>Requirements:<\/h4>\n<ul>\n<li>A professional degree in pharmacy, medicine, or a health-related discipline; OR a baccalaureate or master\u2019s degrees in related fields such as epidemiology, economics, or public health.<\/li>\n<li>Unofficial transcripts showing a minimum grade point average of 3.0\n<ul>\n<li>Official transcripts will be requested and verified by the Graduate School, only if offered admission.<\/li>\n<\/ul>\n<\/li>\n<li>A statement of personal background and goals<\/li>\n<li>A curriculum vitae or resume<\/li>\n<li>Three letters of reference<\/li>\n<li>GRE scores (required for all applicants)<\/li>\n<li>When requested, completion of a personal interview with the Graduate Admissions Committee<\/li>\n<li>English language proficiency (ELP) is required for graduate study at the University of Washington. Therefore, every applicant whose native language is not English must demonstrate English language proficiency.\n<ul>\n<li>For a comprehensive list of how to fulfill the English Language Proficiency requirement, please review <a href=\"https:\/\/grad.uw.edu\/policies\/3-2-graduate-school-english-language-proficiency-requirements\/\">Graduate School Policy 3.2.\u00a0<\/a><\/li>\n<li>For those demonstrating proficiency through the use of test scores, please note that our department has a higher minimum score than the Graduate School. The minimum requirement for possible entrance to our program is a score of <strong>100 on the TOEFLiBT or TOEFL MyBest<\/strong>,<strong> Academic IELTS score of at least 7.0, or DuoLingo score of 130<\/strong>. The Graduate School states a lower score, but individual departments may adjust that for their requirements.<\/li>\n<li>Note: The ETS code for the University of Washington is 4854<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<p>An applicant with exceptionally strong qualifications, such as a strong record of related work experience, who does not meet all of the above requirements may be considered for admission. Plans to remedy academic deficiencies may be suggested either prior to or during the first year of the program.<\/p>\n<h3>Application Process<\/h3>\n<p>Due to the scheduling of core courses, students are accepted for <strong>Autumn quarter only<\/strong>. Autumn quarter begins the last week of September.<\/p>\n<p>Applicants must apply for admission through the <a href=\"https:\/\/grad.uw.edu\/prospective-students\/how-to-apply\/apply-now\/\">UW Graduate School application for admission<\/a>. CHOICE faculty and staff will access the necessary application material from the Graduate School application process for review. No paper materials should be mailed to the department, all materials are submitted online.<\/p>\n<ul>\n<li>Top candidates will be contacted in January if invited for an interview.<\/li>\n<li>Applicants will be notified of admission decisions by mid-February.<\/li>\n<\/ul>\n<h4>How to Apply:<\/h4>\n<ol>\n<li>Login to the Graduate School application:\u00a0<a href=\"https:\/\/apply.grad.uw.edu\/portal\/gr_app\">https:\/\/apply.grad.uw.edu\/portal\/gr_app<\/a><\/li>\n<li>Select \u201cStart Graduate Application\u201d<\/li>\n<li>Complete all sections of the application along the lefthand panel, starting with the Profile Information section.<\/li>\n<li>Under Program Selection, choose the following:\n<ol>\n<li>Application type: Graduate<\/li>\n<li>Program: Pharmacy \u2013 Health Economics &amp; Outcomes Research (PhD)<\/li>\n<\/ol>\n<\/li>\n<li>Review the Program Requirements and indicate the term and year you intend to enroll in the program<\/li>\n<li>Continue with completing all sections of the application, including Application Materials, self-reported GRE scores, and Recommenders.<\/li>\n<li>Review your application and check for any missing required fields or errors. When ready, click Submit Application. Your application is\u00a0<em>not submitted<\/em>\u00a0until you have paid the application fee.<\/li>\n<\/ol>\n<p><strong>Application Fee Waivers<\/strong><\/p>\n<p>The Graduate School application fee is $90.
