      Project Goals<\/strong><\/p>\n

      1. Improve influenza, pneumococcal, pertussis, and herpes zoster immunization rates in adults<\/li>\n
      2. Implement innovative methods for sharing immunization information across appropriate healthcare partners<\/li>\n
      3. Align incentives between payers and pharmacies to identify an effective value-based payment model<\/li>\n
      4. Improve vaccine preparedness planning among the state health department and community pharmacies<\/li>\n<\/ol>\n

        Important Findings<\/strong><\/p>\n

        The project found a 14% increase in the number of adults immunizations in a one year periodas a result of the pharmacist outreach and support.<\/p>\n

        Sponsorship:<\/strong><\/p>\n

        This project was supported by the Center for Disease Control and Prevention (CDC) and the National Association of Chain Drug Stores (NACDS). This project is part of a larger initiative with three (3) sub-awardees spanning four (4) states to increase adult immunizations.\u00a0 The demonstration projects are supported by Grant Federal Award Identification Number, IH23IP000985, for $800,000 funded by the CDC. NACDS contributed an additional $300,000 to execute the demonstration projects.\u00a0 Given this contribution, each sub-awardee received approximately 67 percent ($200,000) of its funding from the CDC and approximately 33 percent ($100,000) from NACDS to implement its demonstration project. A total of $1.1 million will be used to complete CDC project work, with 73 percent from the CDC and 27 percent from NACDS.\n<\/p>\n<\/div>\n<\/div>\n
