Lock out padlocks are issued to tradesmen so that they can shut down the supply of water or electricity to equipment they will be working on. Each lock must display the identity of the lock’s owner and a trouble phone number.

Loto Issue Procedure

Requests for lock out tag out sets arrive via email from shop supervisors.

The message should include the employee’s name, eid number and crew.

To issue a lock set you will need:

The spreadsheet “lockout key assignments”.

A new lock set of four padlocks.

A set of four brass id tags for the locks

Tools to affix the tags to the locks.

To issue a lock set

1. Upon receiving a request from a supervisor for a lockset, send a request for engraved brass tags to Shauna Burnstein in the sign shop. Email the request attaching a spreadsheet with these column headings:

Name, client eid, phone (206-685-1400), quantity (4ea).

2. Pull the next set of locks in sequence from stock and fill out the fields in the “lockout key assignments” spreadsheet.

3. When the tags arrive, (sign shop will deliver), assemble the locks per procedure.

4. Enter the issue into the lending tool log book and note the name of the expediter who will deliver the set and notify the dispatcher.

5. Charge the cost of the lock set to the crews overhead number using a counter release.
